Consumer Reporting Potential Clients

Wells Law - Chicago created this form to help potential consumer reporting clients stay organized and on track throughout the evaluation process.


The documents and information you provide here are critical evidence. They will help us determine whether we can offer representation and, if so, support our ability to pursue the strongest possible outcome on your behalf.


This form is primarily for your benefit. Feel free to update it as new information becomes available. If you have any questions as you work through it, please contact us.

1. Detailed Timeline

A typed, chronological account of events (i.e., who, what, when) from the beginning, updated as new developments occur.

2. Unmarked Consumer Reports

Unmarked copies of each credit report that lists inaccurate or incomplete information (for potential evidentiary use).

3. Consumer Report with Notes & Highlights

Copies of your reports with each error flagged and labeled (e.g., “*-A”), along with a separate document explaining each error.

4. Your Disputes

Copies of your written disputes to any furnisher, creditors, employers, or credit agencies. If disputes were verbal, include them in your timeline with as much detail as possible.

A "furnisher" of infomration is any entity who provides the information to the consumer reporting agency that reports it about you. For example, it may be a bank, employer, landlord, insurance company, etc.

5. Responses to Your Disputes

Any written responses, plus details of any verbal responses included in your timeline.

6. Third-Party Contacts

Documentation, or timeline notes, for any communications with agencies such as the Attorney General, BBB, FTC, CFPB, or law enforcement about the errors on your report.

7. Adverse Actions

Records of any denials of credit, higher rates, job refusals, etc., related to the errors (or timeline notes if verbal).
